The error you are getting is a generic error that may appear while using Kayako Classic if a field on a form that requires a value was left empty or blank. The solution is to simply make sure all required fields of a form are filled out before submitting or updating the form, in this case, the report creation form.

In the above example, it appears that there are no categories added to the report. Hence you need to create a new category under Staff CP > Reports > Categories > New.
